What is a Support Coordinator?

Support Coordination is an NDIS funded support for people who need extra help implementing their plan. This could be because your plan is more complex and you require a bit more support to get what you need.

A Support Coordinator works in a similar way to a Local Area Coordinator and will coordinate services from a range of suppliers or providers, address service delivery issues and develop the capacity and resilience of your support network.

A Support Coordinator can also provide case management support if required. Support Coordinators generally work at a coordination of supports or specialist support coordination level.

The NDIS describes Support Connection as providing assistance for participants to implement their plan by strengthening the ability to connect with the broader systems of supports and understand the purpose of the funded supports and participant in the community.

Support Network

A facilitated, informal circle of  friends and supporters. 

Helping those with disability to achieve their personal goals, enjoy friendships and social outings, and be more inclusive in their community.

Tenancy & Accomodation

Plan and transition to independent living. We provide advice and support on buying property, rent and tenancy obligations, shared living arrangements, and disputes. 

These services can be included in an NDIS Plan.

Support Connection

Building participants’ ability to connect with informal, community and funded supports. 

We can help implement funded supports in plans and link participants to community, mainstream and other government services.
